We'll look at why care plans succeed or fail on logistics more than motivation, and what that means for how we build them. You'll leave with practical tools for:

  • Starting from a person's actual daily rhythms and existing routines, rather than an ideal version of their life

  • Spotting the physical, cognitive, and emotional friction that quietly derail good plans

  • Designing a "bad day" version of any protocol, so the plan still holds when life doesn't cooperate

  • Making it easier to remember and follow through, by habit-stacking, visibility, and borrowed structure

  • Treating a care plan as a living experiment, refined in conversation, instead of a fixed protocol
